The Rock Island High School Athletic Department is offering $5 Sports Physicals for the 2019-2020 school year to Junior High and High School students on Tuesday, June 4.
Includes students in 6th grade who will be in 7th grade for the 2019-2020 school year.
High School student sports physicals will be held from 6 - 7:30 p.m. Junior High School student sports physicals will be held from 7 - 8:30 p.m. Students are to enter at the Field House doors.
Students need to bring the IHSA Physical Form (parents MUST sign) - PDF below. Students will not be able to have the Sports Physical without the signed form!
