Your elementary school is participating in an important public health program, Flu-Free QC which is available through Genesis Health System.

With your consent, your child(ren) may receive a free flu shot during the school day from a Genesis Visiting Nurse. The goal of the program is to protect your child from getting the flu and keep your family safe and healthy as well.

To allow your child(ren) to participate with their class, simply complete the consent form attached below or the online consent form at the Genesis website: www.genesishealth.com/flufree. Please review the Vaccine Information Statement (VIS) to answer any questions you may have about the flu vaccine prior to signing the consent form.

This program does not include a second clinic at your elementary school for those children that may require a second vaccine. The information sheet sent home with your child will help you determine whether your child will need a second dose of flu vaccine. Your local health department or primary care physician will be able to assist you as well.
