Public Use of District Facilities: The Board of Education believes that the school facilities and grounds of the School District should be made available for community purposes.

The Board permits the use of school facilities and grounds only when such permission has been requested in writing within reasonable timelines and has been approved by the building principal and district comptroller.

Please complete the form and submit to the building principal. A Certificate of Insurance listing the Rock Island-Milan School District #41 as additional insured is required. Coverage limits: $1,000,000 general liability occurrence, $2,000,000 general liability aggregate and $500,000 property damage.
